2019-2020 School Year Forms & Information

Hello Carey Families,

Summer break is right around the corner. In preparation for the start of the 2019-2020 school year, please read through all of the following information paying especially close attention to the section below, Online Forms to Review and Update.

We hope that you and your family enjoy the summer break!

All mandatory online forms need to be submitted by Friday, June 14th, 2019.


Online Forms to Review and Update

It is The Carey School's policy for parents to review and update family and child information every year.  Much of the information collected is vital to the safety of every person on campus. For returning families, forms have been pre-populated where applicable with information from last year. Please review and update all forms with any changes. It is mandatory to review all online forms.

NOTE: Your child will not be allowed to start school without filling out and submitting all mandatory forms.


Instructions for Completing Online Forms

1. Open School Forms Online

  • Review, complete, and submit the forms located under your Child's Name.
  • Review, complete, and submit the forms located under Family.
  • IMPORTANT: All mandatory forms must be submitted even if the information has not changed since last school year.

2. Open Family Access Module

  •  Review, complete, and submit family information by clicking on the navigation tabs labeled:
    • Household
    • Parent A
    • Parent B
    • Emergency (includes student pickup authorization)
  • IMPORTANT: All mandatory forms must be submitted even if the information has not changed since last school year.


We've provided a helpful video that demonstrates the location of the Family Access Module forms.



NOTE: You may access the Family Access Module throughout the school year to update any information that changes.

Should you have any difficulties accessing our forms, please email the Office of Information Systems & Technology for support.

Physical Forms to Hand in to Our Front Office

Health forms have been mailed for completion to families with new students. The forms below should be completed and submitted by hand or emailed to our front office (if applicable).

Authorization for Medication Administration - if applicable

Anaphylaxis Emergency Care Plan - if applicable

P.E. Clothing

Carey School students should come to school dressed in clothing appropriate for P.E. days.

Appropriate attire for P.E. includes:

  • Red or white athletic shorts, skorts, sweatpants, track pants, or leggings
  • Red or white t-shirts (either short or long sleeves); or Walkfest t-shirts
  • Athletic socks (any color)
  • Athletic shoes (any color)
  • During cold weather, students may also wear red or white jackets or sweatshirts

Why do we have a P.E. clothing policy?
P.E. uniforms allow our children to focus on exercising during P.E. class.

Where can I buy P.E. clothing?
Any red and white athletic clothing is fine for P.E. days. However, for your convenience, P.E. appropriate red and white athletic wear, either with or without the school logo, may be purchased from Lands’ End.

Children in Pre-K and Kindergarten are encouraged to have one shirt with the school logo for identification purposes when on school field trips.

How do I order P.E. clothes from Lands’ End?

  • Visit visit Land’s End website and click on “Shop by School”
  • Enter the relevant information and click “Search by Address”
  • Any matching school selections will appear on the right; Select School
  • If you wish to create an account, put in information and click “Save School”
  • Create or enter your account log-in information and start shopping
